Personal Release

Focus group release forms...

We decided that we should use Personal Release forms for the people used in our Dolls Domain Focus Group, this is because it allows the people to withdraw their information from our group. It is a simpler way than using questionnaires. We used them as in the real world, professionals would have to sign these to show they have given consent.

Our cast release form...

The reason we use a personal release form for our character was because they needed to sig tone able to stay in contract, also to be able to withdraw their information. This signed paper gives us permission to se the character in film and to be exposed to others. We had to have parent sign this form as the girl was under 18 (being me) and so she is vulnerable to exposure.
